Rasadushti- Pandu (Hypothyroidism)- a case study

The case-

On 17th July, 2011, an 11-year-old girl was brought to clinic, diagnosed as having hypothyroidism. Her report on 12th July, 2011 from Thyrocare was
 T3- 38.06, T4- 0.30, TSH > 150.
 Her report on 20th June, 2011 from SRL lab was
antithyroglobulin antibodies- 131.80, and
Thyroid peroxidase antibody- 166.20.
As per experts in that lab, she could have Hashimoto's disease.

Lakshana-

As per Ayurvedic terminology, her main symptoms were-

तत्समये नासा प्रतिनाह,
मलप्रवृत्ति प्रायः विबद्ध, तृतीयेऽह्नि,
क्षुधा- अविशेष,
वारंवार पिण्डिकोद्वेष्टन,
श्रम,
क्लम,
आलस्य,
उत्साहहानि- विशेषतः प्रातः, कार्श्य

These symptoms reveal Agnimandya and Rasadushti. Also, Krimi (worms) was another important factor. Here वारंवार पिण्डिकोद्वेष्टन, was due to Rasadushti and Pandutva. It was also due to विबद्ध मलप्रवृत्ति producing Pureesha Vegavarodha like effects. Due to Rasadushti, abnormal Kapha was produced, giving rise to वारंवार प्रतिश्याय. Majority of other symptoms were linked with Rasadushti like श्रम, क्लम, आलस्य, उत्साहहानि, कार्श्य.
 Ayurvedic methods of examination are also very much important.
In this case-

परीक्षण-
नाडी- अपान,
 à¤‰à¤­à¤¯à¤ªुट नासार्श,
कण्ठ- दक्षिणतः अल्प कठिन-चल उत्सेध (enlarged lymph nodes)

जिह्वा- श्वेताभ उपलेप suggested accumulated Saama Doshas due to recurrent Ajeerna.. वामकुक्षि-वंक्षणदेशे आध्मात ध्वनि is indicative of improper evacuation and therefore accumulation of Pureesha, in Uttara Guda.

Hetu- Samprapti-

In Goa, school timings are up to 1.30 pm. By the time children reach home, its already 2.15 pm or more. Many children eat slowly (many of them watch TV programs while eating, and therefore there maybe over eating). By the time they finish lunch, its 3 to 3.30 pm. Many of them then sleep, as they wake up early in the morning. Many children either avoid exercise or playing on the ground. Sometimes there is no place to play. Since they have late lunch and day time sleep, they don’t get good appetite, and eat late in the night. Similar is the routine of most of the teachers. In this girl also similar Hetu were observed. All this results in recurrent Ajeerna, Rasadushti. Since the digestion is not proper, Kitta portion also is Saama. The Pureesha produced from it is also Saama. Therefore, its not evacuated properly, since due to Saamatva, there is Paicchilya(stickiness). Moreover, due to late eating and sleeping just after it, the digestion is incomplete.  Due to recurrent Ajeerna, Rasa Dhatvagni Mandya takes place. Saama Rasa Dhatu results in blocking of Srotas. Therefore, nutrition doesn’t reach further Dhatus properly. Ultimately Pandu is produced. 
Her mother also had modern diagnosis as hypothyroidism. Her mother also had history of recurrent Pratishyaya and Shwasa. She had Rasadushti, resulting in Vikrita Utpatti of Kapha.

Chikitsa-

Considering all these factors,Navya Sootashekhara was given for Amapachana, Rasa Dhatvagni Deepana and also for Prakrtivighata of Krimi. It also removes Srotorodha.Due to manganese Bhasma in it, it provides Bala to Rakta.  Navya Sootashekhara is a specific formulation prepared by late Vaidyaraj Mukundrao Gayakwad, from Solapur, with addition of few more medicines like Tapyahata Tamra, Manganese Bhasma, Jaharamohara Pishti, Kukkutanakhi, Dhamasa and Untakatari(Brahmadandi) Moola Tvak Ghana. Original ingredients of Sootashekhara are not altered.   
 Kukkutanakhi was given for reducing Kapha and also for symptoms like श्रम, क्लम, आलस्य, उत्साहहानि , because it is Medhya and reduces Tamoguna. This was given in I.e. morning evening, so that it will have specific action on Kapha.
Triphala, Katuka and Swarjika Kshara was given in i.e. before meals, so that it will have specific action on Apana and also on Pureeshaja Krimi.
Krimikuthara was given with Kampillaka in the night, as Krimi become more active in the night.
Later on, as she got relief in symptoms, Lakshmivilasa was added to her prescription. Kshara and Krimikuthara were discontinued. Finally, only Tapyadi Loha and Amalaki were continued.
She became symptom free, gained weight and overall health improved. Even her menarche took place and had normal menstruation.
The changes in reports can be noted as below-

      26-11-12   SRL lab- antithyroglobulin antibodies- not detected,
Thyroid peroxidase antibody-  41.40 (< or = 60 negative)

Instead of modern diagnosis and Ayurvedic treatment, when we diagnose the patient as per Ayurveda and then treat, then we can provide better and quicker results. When we try to establish Dhatusamya, the reports also start falling in normal range, though our treatment is not exactly based on those reports.
We have records of several cases diagnosed as having hypothyroidism and then after Ayurvedic diagnosis and treatment, their health is improved and even the reports become normal. Complete Ayurvedic thought process adopted in various patients, can be described in separate articles. We treated the patients who were not on any modern medication yet, fully with Ayurvedic treatment. In the patients who were on modern medications, we started Ayurvedic treatment and then gradually reduced the doses of modern medicine. Later on, they were shifted on only Ayurvedic treatment. In many patients, finally even Ayurvedic medicines were stopped. We have observed that the patients maintained good health even after years.

    1. It is a specific formulation of Vaidyaraj Mukundrao Gayakwad. Presently being bused by his son Vaidyaraj Ramesh Gayakwad. Unfortunately not available in market.
