An effort to understand the mechanism of 'Vaman-virechan-vyapad'

1.      Description of ‘Vyapad’ is the best to understand the reverse pathogenesis when desired action is not performed   by body as a response to drugs induced.

2.      Five fold description mentioned above, is sufficient to understand action of final triggering Nidan or cause that finally be responsible for the mechanism of Dosha-dooshya-sammoorchhana or pathogenesis viz  Jivadan as a result of Teekshna drug in alpa-dosha along with Mridukoshthi.

3.        Pre existing Dosha-dooshya condition as well as body response may be inferred As per the presentation of signs and symptoms either acute, subacute or chronic e..g. Parikartika or Jivadan.

4.      A physician should be able to make hypothesis / identify the said pre-existing Dosha-sthiti as per the basic dash-vidh-parixa etc, and illustrated concept of ‘Vyapad’ must be helpful to understand that. 

5.      This will not only meaningful to assess the severity of clinical presentation but also in identifying/locating the final triggering ‘Nidan’ that is to be restricted as per principal of Nidan-parivarjanam’.

6.      Said concept may be utilized in the prediction/future illness, exact prognosis of the disease (along with the concept of ‘Indriyasthan’) as well as emergency management.

7.      As per my understanding that without the basic approach of said concept of ‘Vyapad’, it will be difficult to handle the emergencies in Ayurveda. It should not be a false statement If we say that said concepts are the basics principles of emergency-medicine.

8.      This should be noted that during the said description Acharya Charak has not mentioned the name of any disease where ‘Shodhan’ to be administered.

9.      He fully targated the area of  ‘Koshtha’/‘Agni’ as per the importance of Agni in Ayurveda during said description that’s why improper Panchakarma procedures have been categorized as ‘Nidan’ of various ailments.

10.  Illustrated concept emphasizes  that how pre-existing silent/asymptomatic doshaja conditions are triggered and lead to final clinical presentation either acute or sub acute  in healthy as well as well diagnosed complicated patients as per Desh, Kaal, Koshtha,Agni, Aushadhi etc.  

11.  It should also be noticed that Acharya has not mentioned so intense emergency management in any other disease except the concept of ‘Vyapad’ where all possible best measures have been applied to save the life of patient.


12.     Presentation of signs and symptoms as per the involvement of  Srotas, Koshtha, Shakha etc may be understood as per the specific responsible Nidan etc.
